How does Dexlan 30mg Capsule work?

Dexlan 30 is a proton pump inhibitor (PPI). It works by reducing the amount of acid in the stomach which helps in the relief of acid-related indigestion and heartburn.

Side Effects of Dexlan 30mg Capsule

• Flatulence
• Diarrhea

• Dexlan 30mg Capsule should be taken 1 hour before a meal, preferably in the morning.
• It is a well-tolerated medicine and provides relief for a long time.
• Avoid eating late at night or before bedtime.
• Inform your doctor if you get watery diarrhea, fever, or stomach pain that does not go away.
• Inform your doctor if you do not feel better after taking it for 14 days as you may be suffering from some other problem that needs attention.
• Long-term use of Dexlan 30 can cause weak bones and a deficiency of minerals such as magnesium. Take adequate dietary intake of calcium and magnesium or supplements as prescribed by your doctor.
• Consult your doctor right away if you develop decreased urination, edema (swelling due to fluid retention), lower back pain, nausea, fatigue, and rash or fever. These could be signs of a kidney problem.

Indication of Dexlan 30mg Capsule

Dexlansoprazole is indicated -

• For the healing of all grades of erosive esophagitis (EE) for up to 8 weeks.
• To maintain healing of EE and relief of heartburn for up to 6 months.
• For the treatment of heartburn associated with symptomatic non-erosive Gastroesophageal Reflux Disease (GERD) for 4 weeks.

Pharmacology

Dexlansoprazole delayed-release capsule is a proton pump inhibitor (PPI) that inhibits the (H+/K+)-ATPase in the gastric parietal cell, suppressing gastric acid output. Dexlansoprazole works by blocking the final step of acid generation by acting on the proton pump. It's lansoprazole's R-enantiomer (A racemic mixture of the R- and S-enantiomers). Dexlansoprazole is available in capsule form with a Dual Delayed Release (DDR) formulation for oral consumption. This capsule contains a mixture of two types of enteric-coated granules with different pH-dependent dissolution profiles. The dual delayed-release formulation in Dexlansoprazole has a plasma concentration-time profile with two distinct peaks; the first peak occurs 1 to 2 hours after administration, followed by a second peak within 4 to 5 hours. After oral administration, the mean Cmax and AUC value of Dexlansoprazole increased approximately dose proportionally. Dexlansoprazole is processed extensively in the liver and excreted in the urine.

Dosage & Administration of Dexlan 30mg Capsule

Adult Dose:

• Erosive Esophagitis Healing (capsule): 60 mg orally once daily for up to 8 weeks
• Erosive Esophagitis Maintenance (capsule or DR): 30 mg orally once daily for up to 6 months
• Gastroesophageal Reflux Disease: 30 mg orally once daily for 4 weeks

Hepatic Impairment:

• Mild (Child-Pugh A): No dose adjustment is necessary
• Moderate (Child-Pugh B): Not to exceed 30 mg/day
• Severe (Child-Pugh C): Not recommended

Child Dose (above 12 years):

• Erosive Esophagitis Healing (capsule): 60 mg orally once daily for up to 8 weeks
• Erosive Esophagitis Maintenance (capsule or DR): 30 mg orally once daily for up to 6 months
• Gastroesophageal Reflux Disease: 30 mg orally once daily for 4 weeks

Dexlan 30mg Capsule should be taken 1 hour before a meal, preferably in the morning. Swallow the capsule whole with water, and do not crush or chew it. For children under 12 years old, safety and efficacy have not been established.

Interaction

• Concomitant use of dexlansoprazole with certain medications should be avoided or closely monitored to prevent potential interactions:
• Anti-viral drugs like Rilpivirine or Atazanavir, and other anti-HIV medications, such as dexlansoprazole may affect their efficacy.
• Blood-thinning medications like Warfarin or Phenprocoumon, as combining them with Dexlan 30mg Capsule may lead to increased bleeding, which could be fatal.
• Methotrexate: Long-term use of dexlansoprazole along with methotrexate may have a toxic effect.
• Medicines dependent on gastric acidity for absorption, such as iron salts, erlotinib, dasatinib, nilotinib, mycophenolate mofetil, ketoconazole, and itraconazole. Dexlansoprazole reduces the absorption of these medications.
• St. John's Wort: Inform your doctor if you are taking this herbal preparation, as it may decrease the efficiency of dexlansoprazole.
• Water pills (diuretics) like Bumetanide (Bumex), and Furosemide (Lasix): Close monitoring is advised as diuretics can cause magnesium imbalance, which may be worsened by dexlansoprazole.
• Discuss with your doctor any other medicines, herbal preparations, and supplements you are currently taking to avoid potential drug interactions. This will ensure the safe and effective use of dexlansoprazole and other medications.

Contraindications

Pregnancy & Lactation

Pregnancy:

• There are no specific studies on dexlansoprazole use in pregnant women to determine any drug-associated risk.
• Dexlansoprazole is the R-enantiomer of lansoprazole, and observational studies of lansoprazole use during pregnancy did not show any adverse pregnancy-related outcomes.
• Animal studies with oral lansoprazole administration to rats during organogenesis through lactation, at 1.8 times the maximum recommended human dexlansoprazole dose, showed reductions in certain offspring parameters like femur weight, femur length, crown-rump length, and growth plate thickness (in males only) on postnatal day 21. These effects were associated with a reduction in body weight gain. Pregnant women should be informed about the potential risk to the fetus.

Lactation:

• There is no available information on the presence of dexlansoprazole in human milk, its effects on breastfed infants, or its impact on milk production.
• However, lansoprazole and its metabolites have been found in rat milk.
• When considering dexlansoprazole use in lactating women, the developmental and health benefits of breastfeeding should be weighed against the mother's clinical need for therapy and the potential adverse effects on the breastfed child, both from the treatment and the underlying maternal condition.

Precautions & Warnings

• Eat smaller meals more frequently.
• Maintain a healthy weight through regular exercise.
• Avoid lying down after eating to prevent acid reflux.
• Practice relaxation techniques and reduce stress with yoga or meditation.
• Avoid tight-fitting clothes that can increase abdominal pressure.
• Avoid certain foods like high-fat, spicy foods, chocolates, citrus fruits, pineapple, tomato, onion, garlic, tea, and soda.
• Take short breaks during prolonged sitting by doing brisk walking or stretching.
• There is limited information about the use of dexlansoprazole in pregnant and lactating women. Pregnant and breastfeeding women should consult their doctors before using Dexlan 30mg Capsule.
• Avoid smoking and alcohol consumption, as they can increase stomach acid production and worsen acidity and heartburn.
• There is no data available on the presence of dexlansoprazole in human milk and its effects on breastfed infants. While lansoprazole and its metabolites have been found in rat milk, the developmental and health benefits of breastfeeding should be weighed against the mother's clinical need for therapy and potential adverse effects on the breastfed child.
• Before starting Dexlan 30mg Capsule therapy, the possibility of gastric malignancy should be ruled out, especially in patients with new or worsening symptoms, or those who do not respond to treatment.
• Use of Dexlan 30mg Capsule in patients with hepatic impairment should be done with caution. Dose adjustments may be necessary based on the severity of the impairment.

Inform your doctor before taking Dexlan 30mg Capsule if you are due to have a Chromogranin A blood test, as it may interfere with the results.

Regularly consult your doctor to assess bone mineral density, as long-term use of Dexlan 30mg Capsule may affect bone health. Regular monitoring of blood and liver enzyme values may be necessary during therapy.
